Three HCA hospitals in the Houston metro have piloted a small but unusual retention protocol over the past nine months: every nurse on every shift gets a guaranteed 20-minute, off-the-floor sit-down breakfast or dinner, paid, with charge nurses accountable for protecting the time.

Nursing turnover at the three pilot sites dropped 14% over the period, against a system average of -1%. HCA says it is now considering wider rollout.
